Submission Requirements
All insurance information and a W-9 should be included with contractor bids. MCHA requires proof of current insurance before award.
Insurance Requirements

Commercial General Liability: $1,000,000 per occurrence / $2,000,000 aggregate

Automobile Liability: $1,000,000 (must include owned, hired, and non-hired vehicles)

Workers Compensation: $100,000
-
All employees of the contractor and any subcontractor must be covered.

If proof of Workers Compensation is not provided, the bid will be rejected and not considered for award.
